Candidates must submit the appropriate fee with a complete examination application. Payment may be made by credit card (MasterCard, Visa, American Express and Discover accepted), personal check, cashier's check or money order.

Forfeiture of Application Fees
A candidate who
- does not schedule an examination within the 90-day eligibility period,
- who fails to reschedule an examination within four business days prior to the scheduled testing session,
- who fails to report for an examination appointment, or
- who arrives more than 15 minutes late for an appointment
will forfeit the examination fee and must reapply for examination by submitting a new application, documentation and full examination fee.
